Pewaukee School District Requirements

1. Completed Physical or alternate year card
2. Signed Handbook card
3. Signed Drug sheet
4. Paid $50 Athletic User Fee

These items much be completed and turned into athletic director's office prior to the first day of practice.

Players who have participated in a fall or winter sport this school year and have completed the above cards/sheets need only pay the $50 athletic user fee.

The $50 athletic user fee check is payable to "Pewaukee Public Schools" and turned into Karen Zitzke, Pewaukee Athletics Secretary.

Questions on the physical/alternate year card, handbook card, drug sheet, or the athletic fee should contact Karen Zitzke, Pewaukee Athletics Secretary.

Frosh/Soph Level-Black hat, Black socks

JV Level-Black hat, Black socks, Black t-shirt

Varsity Level-Red and Black hats, Red and Black socks, Red and Black t-shirts, Red game jersey

There is a link to the apparel order form on the news section of the website.

Orders and payment are due the night of the parent meeting.  If you

cannot attend please return the order form to Coach Dobberstein's mailbox at

the high school.
