Pewaukee School
District Requirements
1. Completed Physical or alternate year card
2. Signed Handbook card
3. Signed Drug sheet
4. Paid $50 Athletic User Fee
These items much be completed and turned into athletic
director's office prior to the first day of practice.
Players who have participated in a fall or winter sport this
school year and have completed the above cards/sheets need only
pay the $50 athletic user fee.
The
$50 athletic user fee check is payable to "Pewaukee Public
Schools" and turned into Karie Williams,
Pewaukee Athletics Secretary.
Any questions on the physical/alternate year card, handbook
card, drug sheet, or the athletic fee, please contact Karie
Williams.

Orders and payment are
due the night of the parent meeting (March 3rd).

the high
school.
